ENROLLMENT PROCEDURES
Documents needed
- Child’s Birth Certificate
- Child’s official Baptism Certificate and 1 COPY OF SAME
- Child’s Immunization Record (up to date with month/day/year) and 1 COPY OF SAME
- Parent’s Marriage Certificate
- Child’s Social Security Number
In addition Grades 2-8 are also required to have:
- Most recent Report Card
- Standardized Test Reports in Reading and Math.
Registration Fee: $75.00 Non-Refundable, Payable at registration time
Registration for Extension Program is available upon request
